NEW STUDENT FORMS
1. Pre-Participation Evaluation
All incoming student-athletes must have completed a pre-participation physical evaluation by the Well Center or a Sac State Team Physician prior to their participation in any officially sanctioned Sacramento State team practices, events, or conditioning sessions.
This evaluation includes:
• Self-completed medical history form
• General medical exam
• Orthopedic medical exam
• Lab work
Pre-Participation physical evaluations will be scheduled and performed at the Student Health Center ($30 fee) or with a Sacramento State team physician (or designated physician). All pre-participation physicals will be done during the summer or prior to the start date of the student-athletes participation. A member of the Athletic Training staff will assist in scheduling the physical appointments.
Any missed physical appointments at the Student Health Center will be subject to a $20 charge.
2. All New Student-Athlete Forms
All new student-athletes must complete their forms prior to any Sac State related activity. Forms are accessible via The WELL’s website via the student-athlete’s patient portal after they have their SacLink login information.
These forms must be completed prior to the student-athlete’s first team meeting with the compliance coordinators and the athletic training staff prior to the start of the sport season.
If the new student will be a minor during any part of the school year please fill out the forms below by the student and their guardian and bring to the athletic trainer in charge of their sport.

If any student-athlete has had an injury or illness which required surgery or ongoing care, please include copies of documentation regarding treatment (operation notes, clinic notes, lab reports, physical therapy notes, etc.)

RETURNING STUDENT-ATHLETE FORMS
All returning student-athletes are required to complete forms assigned by the athletic trainer in charge of their sport. Forms are on The WELL’s website via the student-athlete’s patient portal. Student-athletes will be cleared to participate only after review of all documents by our staff athletic trainers and, if necessary, team physicians.
